ASSUMPTION OF RISK

I understand that participating in golf and using indoor golf simulators involves inherent risks. These risks may include, but are not limited to, being struck by a golf club or golf ball, slips and falls, strains or other physical injuries, contact with equipment or other participants, and injuries resulting from improper use of the facility or equipment.

I voluntarily choose to participate and knowingly assume the risks associated with using the golf simulators, golf clubs, balls, hitting areas, seating areas, and other equipment or facilities at Caddy Shak Indoor Golf Club, LLC.

RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by law, I release and hold harmless Caddy Shak Indoor Golf Club, LLC, its owners, members, employees, agents, representatives, and affiliates from claims for personal injury, property damage, loss, or other damages arising from my participation in activities or use of the facility, except to the extent such a release is prohibited by applicable law.

I understand that this agreement is intended to include claims arising from ordinary negligence to the extent such claims may legally be waived.

SAFETY & FACILITY RULES

I agree that I will:

• •Only swing a golf club when the designated hitting area is clear.
• •Remain outside another player's swing area.
• •Direct all golf shots toward the designated simulator screen.
• •Follow all posted rules and reasonable instructions from Caddy Shak staff.
• •Not engage in horseplay, reckless swinging, throwing clubs, or other unsafe conduct.
• •Immediately stop using equipment and notify staff if equipment appears damaged or unsafe.
• •Use Caddy Shak's facilities and equipment only for their intended purposes.

DAMAGE TO EQUIPMENT OR PROPERTY

I understand that I may be held financially responsible for damage caused by my intentional, reckless, or negligent misuse of Caddy Shak property or equipment, including simulator screens, sensors, projectors, computers, golf clubs, furniture, fixtures, and other equipment.

ALCOHOL & IMPAIRMENT

I understand that alcohol, drugs, medications, or other substances may impair judgment, balance, and coordination.

I agree not to participate if I am unable to safely use the golf simulators.

Caddy Shak Indoor Golf Club, LLC reserves the right to stop participation or require a guest to leave when staff reasonably believes that person's conduct creates a safety risk.

PERSONAL PROPERTY

I understand that Caddy Shak Indoor Golf Club, LLC is not responsible for lost, stolen, or damaged personal belongings except where responsibility cannot legally be excluded.

MEDICAL EMERGENCIES

If I become injured or experience a medical emergency while at the facility, I authorize Caddy Shak staff to contact emergency medical services when reasonably necessary.

I understand that I am responsible for my own medical expenses except where otherwise required by law.
